My father is a nurse and he suspects that I have hypothyroidism. Due to my sensitivity to cold, irregular periods, slight weight gains and tiredness, he says that I have developed this condition. I am only 17-years old and am worried if he’s right. Someone please tell me what are the hypothyroidism diagnosis?

Why not get your Thyroid Stimulating Hormone levels checked. This will be elevated in Hypothyroidism. This requires 12 hours' fasting, so have an early supper(by 8pm) and give your blood sample at the lab by 8am the next day!
If it is indeed elevated, you will need to consult an Endocrinologist.
