Description

Pinom C Tablet is a combination of Cilnidipine and Olmesartan. It is used in the treatment of Uncontrolled Hypertension (high blood pressure). This medicine decreases your heart rate and relaxes the blood vessels so that blood can flow more easily throughout your body, thus controlling your blood pressure. Pinom C Tablet can cause side effects like stomach pain, swelling in your hands and feet, irregular heartbeats, headache, etc. Consult your doctor if these side effect persists or worsen. This medicine can cause dizziness, therefore avoid activities that require mental focus like driving or operating machines. Pinom C Tablet can be taken with or without food, as prescribed by your doctor. Your doctor will decide the dose based on your condition. Take Pinom C Tablet at the same time every day to maintain a constant level of this medicine in your blood. Avoid the discontinuation of this medicine without consulting your doctor. Pinom C Tablet is not recommended if you are allergic to it. Inform your doctor if you have any heart, liver or kidney problems. Inform your doctor if you are taking any other medicines or supplements as they may interact with each other and increase the risk of side effects. Pinom C Tablet is not recommended in children below 18 years, if you are pregnant, or are breastfeeding unless prescribed by your doctor.

What is it prescribed for?

Uncontrolled hypertension

Hypertension or high blood pressure is a condition in which the circulating blood exerts high pressure on the walls of your blood vessels. If left untreated, it can lead to various complications like stroke and heart attack. Pinom C Tablet is used in the treatment of uncontrolled hypertension which is an increase in the blood pressure (where BP >140/90 mmHg), caused by genetic and/or environmental factors.

Allergy

Aliskiren

Aliskiren is a medicine that is used to reduce high blood pressure. Taking Pinom C Tablet and Aliskiren (a medicine used to reduce blood pressure) together is not recommended as it may lead to kidney damage, low blood pressure and hyperkalaemia (high potassium levels in the blood). This risk is especially higher if you have diabetes or kidney problems.

Heart diseases

Pinom C Tablet is not recommended for use if you have any heart problems. This includes heart attack (a condition that occurs when blood flow to the heart is interrupted by a blockage or blood clot), rhythm disorders (abnormal heartbeats), angina (chest pain), etc. This medicine weakens the contractions of your heart and reduces your blood pressure. Using it while you have heart problems will worsen your condition.

Severe liver impairment

Pinom C Tablet is not recommended if you have severe liver problems. This medicine may accumulate in the liver and further lead to liver damage and increased risk of side effects.

General warnings

Aortic Stenosis

Aortic stenosis is narrowing or blockage of the aortic valve that regulates blood flow in the chambers of your heart. Pinom C Tablet should be used with caution if you have aortic stenosis. This medicine slows down the blood flow which may increase the risk of very low blood pressure. It is recommended that you monitor your blood pressure regularly while you are on treatment with this medicine.

Use in Children

Pinom C Tablet is not recommended for use in children below 18 years of age since the safety and efficacy of use are not clinically established.

Hyperkalemia

Hyperkalemia is an increase in potassium levels in your blood. Pinom C Tablet may increase the potassium levels in your blood. This may cause side effects such as irregular heartbeats. It is advised to avoid potassium-rich foods (potatoes, bananas) and other medicines that may increase potassium levels (ACE inhibitors and potassium supplements) during treatment with this medicine.

Coronary artery diseases

Coronary artery disease is a condition where the blood vessels of your heart are damaged, mainly due to the build-up of plaque. Pinom C Tablet should be used with extreme caution if you have coronary artery disease. It may weaken the contractions of your heart and slow down the heart rate, which may further depress cardiac output (amount of blood pumped by the heart in a minute) and blood pressure. Your doctor will suggest tests to closely monitor your heart function while you are on treatment with this medicine.

Driving or operating machines

Pinom C Tablet may cause symptoms such as drowsiness and/or dizziness. Avoid performing activities that require high mental alertness like driving vehicles or operating heavy machines during treatment with this medicine.

Drug withdrawal

Avoid abrupt discontinuation of Pinom C Tablet as it may worsen the condition such as angina (chest pain). Your doctor may gradually reduce the dose based on your condition to avoid undesirable effects.

Missed Dose

If you forget to take the dose of Pinom C Tablet at a given time, then take it as soon as you remember. If it is time for your next dose, skip the missed dose. Do not double your dose to make up for the missed dose.

Overdose

Never take more than the dose prescribed by your doctor. If you have intentionally or accidentally consumed more than the prescribed amount of Pinom C Tablet, contact your doctor immediately for further guidance.

Hypotension

Kidney Disease

Liver damage may lead to the accumulation of this medicine and increase the risk of side effects. Food interactions

Consumption of grapefruit juice is not recommended during treatment with Pinom C Tablet as grapefruit juice can increase the absorption of this medicine in your blood. This could lead to excessive lowering of blood pressure or worsen side effects.

Urinary vanillylmandelic acid

Urinary vanillylmandelic acid test is performed to determine the presence of certain tumours. Inform your doctor or the lab technician about the use of Pinom C Tablet as this medicine is likely to give a false-positive result.
Pinom C Tablet may be taken orally with or without food. Do not crush, break or chew the tablet. Avoid taking more or less than the dose prescribed by your doctor. You should not stop taking the medicine abruptly without your doctor's consultation as it can cause withdrawal symptoms. Lifestyle modification, eating a balanced diet, and exercising regularly will help you to have better control over your blood pressure. Avoid taking grapefruit juice with Pinom C Tablet as it can increase the risk of side effects. Using Pinom C Tablet together with multivitamins containing minerals may decrease the effects of this medicine. Keep a two-hour gap between the intake of both medicines. Be careful while standing up from a sitting or lying down position as this medicine may sometimes cause a sudden drop in your blood pressure, which can make you dizzy. This medicine causes swelling in your ankles and feet, so try raising your legs when you are sitting. Consult a doctor if this swelling worries you. This medicine may cause dizziness so try to avoid activities that require mental focus like driving and operating machines.

How it works

Pinom C Tablet contains olmesartan and clinidipine as its major componenets. Olmesartan works by blocking a receptor and preventing blood vessel constriction. Cilnidipine acts by blocking the inflow of calcium (responsible for the contraction of blood vessels) and causes relaxation, thereby promoting the smooth flow of blood. Their combination is thus effective in reducing blood pressure.
