Your report is suggestive of a partial disc prolapse at the level of the neck and the lower back with changes in the normal anatomy and compression of the nerve roots by the disc. Pain medications and neurotopes will only give a limited amount of relief and it doesn't seem it will get better with physiotherapy and medications. I would advise you to meet a spine surgeon/neurosurgeon so that he can be examined and the decision on the surgery can be made.
