I'm trying to understand. Please help :)
Why do Grade 3 ACL degrade with time? Isn't there a way they can attach themselves? or regrow? During surgery why are holes drilled in when the section of the Patella and the tendon can be moved to a certain extent and with that gap stitch the ACL together and allow them to heal? or stitch the ACL from behind the leg where there are only muscles?

ACL once torn , if repaired by either naturally or surgically do not get back the natural strength. But if kept together, the torn ends do Healy to each other by a fibrous mesh. But the quality of healed patch remains questionable.. In case of injury, the ends of year are eaten away for 2-3 mm to initiate repair. In ACL reconstruction, old tendon is removed and new tendon in out in its place by drilling holes in femur and tibia. The ACL cannot be stitched to back muscles as it is a connection between two bones only.

In professional athletes, runners and people with love for sports, reconstruction is better if there is instability .

Static quadriceps exercises strengthen remaining muscles and ligaments to overtake the work of ACL, which is good enough in people with low functional demands.
