Scalp psoriasi all nail severe psoriasis

I have moderate scalp psoriasis and 20 nail psoriasis severely deformed means nail plate from nail bed till cuticle it's very painful I couldn't not tell anyone because my family is not taking serious about me and my disease. Till now I can't get rid of it at all by using all topical medicine. so now situated is my both hand thumb have pain  when I use it hurts a lot it's been few months ago it's started Also seldomly I got pain in my knee not much as my thumb have resist pain which is not going away. My scalp psoriasis build up very quickly in  cause very red ,inflamed and thick flakes I'm losing so much hairs too more than 100. Once doctor have prescribed disease modifying druge Methotrexate , saaz , apremilast, tofacitinumuab, I took for 3 months and had relief after I didn't take so disease come back since may I'm taking apremilast 10mg slowly increase the dose but i got headaches very badly so now I'm on 20mg apremilast once a day after food in the evening time. but no relief yet

Hello dear. You are 25 years old, what was th3 reason for stopping methotrexate? Are you planning conception? It is a good drug in psoriasis, especially because it is cheap. As for tofa, also great drug, but cost is a factor, so not many can afford. Please send pictures so we can determine your %body surface involvement. I am guessing you have psoriasis arthropathy, has any X ray been done? Please attach pictures, if yes
