My voice has been changing for over a year and I have started to get very breathless even just going from sitting to standing up.

Change in voice indicates pathology in the vocal folds.. consult with an ENT specialist for proper assessment and go for fibreoptic laryngoscopy to rule out vocal folds disorder..
